Autism Spectrum Disorder (ASD) is one of the commonest childhood neuro-psychiatric disorders. In the USA the incidence of Autism is 1 in 68 children. The shock of finding out that one’s child has autism is life-changing experience for most parents. Autism is a childhood disorder that results in speech problems, hyperactivity, aggressive behaviour and difficulty in social interaction. There are certain disorders, where despite the best available medical and rehabilitative treatments, till date, medical science has not been able to give the patients satisfactory relief in their medical and physical symptoms or help them to regain the abilities to integrate with society. Autism is one such disorder.

Dr NandiniGokulchandran Deputy Director & Head Medical Services, NeuroGen Brain & Spine Institute said “Autism causes impairments in higher mental functions and causes the children to be hyperactive and aggressive and so affects their social interactions. Speech, language and communication skill are severely impaired.  These children cannot go to normal schools and are completely dependent on parents and caretakers for activities of daily living. Having a child with Autism severely affects the emotional, social and financial aspects of the entire family. It is more commonly observed in boys than girls. The onset of symptoms is generally noted before the age of 3 years in children, although in many cases it is noticed at a much older age. The exactetiology and pathophysiology remains poorly understood, however research has pointed to several possible factors, like genetics (heredity); metabolic or neurological factors, certain types of infections, prenatal or post natal environment, etc. “

Today, here we are presenting a case report of Master AdnanTabranwhois a 7-year-old male and a known case of Attention Deficit Hyperactive Disorder (Autism) from Udaipur. There were complications during the 9th month of pregnancy period of Adnan’s mother, hence Adnan was born 8 to 10 days prior from due date with the help of Induced labor technique. He cried immediately after birth and no complications were reported post that. He achieved all his visual milestones age-appropriately. His motor and speech milestones were delayed. He started walking at the age of 16 months and speech developed in monosyllables at 3 years of age. His arm hand control and personal social development was delayed and achieved at around 3.5 to 4 years of age. “When Adnan’s age was around 2 years, we noticed that he is too hyper and restless and his learning is not appropriate as compared to children of his age. We used to try and control him through different methods but none of the methods worked for more than 5 to 10 mins and again he used to start running around. 2 years ago when Adnan was 5 years old, he was shown to a psychologist in Ahmedabad where he underwent psychological assessment. Based on clinical picture and investigations, he was diagnosed with ADHD and speech delay” said Mr. Tabran, Adnan’s father.

There is No History of Autism or any Psychological Disorder in family. Due to lack of awareness initially for few years Adnan’s family didn’t even realized that this behavior of Adnan is because of a brain condition. After getting Adnan diagnosed from the psychologist in Ahmedabad, Adnan and his mother stayed in Ahmedabad for 3 months to take occupational therapy and speech therapy. The results were not very great, plus the treatment given was minimal and did not receive a proper guidance as to what all measures should they take for his issues. Mrs. Tabran, Adnan’s mother learnt a lot from there and continued with therapies at home after the 3 months’ course. Adnan went to normal school from age 5 to age 7.

Mr. and Mrs.Tabran came to know about NeuroGen from internet when they were searching about different medicine options for Autism (ADHD) in March 2016.  Adnan came to NeuroGen Brain & Spine Institute in December 2016 to undergo Stem Cell Therapy and Nero-rehabilitation.

On examination, the chief complaints found in Adnan were that easy distractibility was present. He used to find it very difficult to maintain his focus on the task.  He followed 1 step simple commands that were usually routine commands. His judgment was affected. His imitation skills were poor. He could only engage in unstructured play. He was uncooperative while playing with other children. Poor game concept was mentioned. Hyperactivity was present. His sitting tolerance was poor. His eye contact was brief. Temper tantrums were present. He tends to bite and scratch and hits when his demands were not met. He faces difficulty in communicating appropriately. Initiation of interaction is difficult. There was presence of irrelevant self talk with varying pitch. His speech was unclear and irrelevant. Memory was below average. For his All Day Living activities, he required assistance for all the activities. He used to go to the toilet on his own but needed help for post-toilet hygiene.

At NeuroGen, Adnan underwent Stem Cell Therapy along with a customized rehabilitation program. The aim of the rehabilitation program was to develop cognition and understanding, therapies to reduce the hyperactivity and to increase the concentration, sensory integration, command following and to increase overall intellectual ability of the patient. He was given exercises that would help him improve his understanding, address his behavior, sensory and motor issues. He was given a combination program with occupational therapy, physiotherapy, speech therapy and psychological counseling under the guidance of the most experienced professionals in the field. This helped him to develop his overall skills, abilities and overall development. These exercises carried out in a systematic pattern with sufficient rest intervals to the patient. Post stem cell therapy, the improvements seen in Adnan are that, his Attention and concentration has improved, pays more attention. Compliance has increased. Follows more command. Time orientation is better. Knows the time of different things. Comprehension has increased. Situational awareness is better. Recognizes all the objects in the house, does color identification, sorting. He is more aware now of vehicles/animals coming from far. Can operate the phone completely. Emotional responses have improved. Sitting tolerance has improved quite a lot. Social behavior/inhibition is better. Eye contact has improved. Looks immediately when called, tries to sustain eye contact. Speech has improved. can speak long sentences now with proper syntax. He conveys more needs with more details verbally. He grasps things very quickly. Attempts to do all the activities on his own. Can perform with verbal prompts. Recognizes his toilet needs. Goes to the toilet on his own. Now he is aware of how to perform tasks. Can put phone on charging. Answers phone calls. FIM score has improved from 75 to 86 in view of selfcare, cognition.

Currently Adnan has been enrolled to a special school where he is personally trained with therapies and special education. He likes going to his school. There is no concept of  shadow teacher in their area hence Adnan had to be removed from the earlier normal school, now in his new school which is a special school gradually his understanding and learning is improving. The overall experience of Adnan and his parents in NeuroGen was very good. They found a proper guidance to address all his issues. Adnan now goes for occupational therapy and special education every day.
